Your Visit

We are pleased that you have chosen the Cosmetic Eye Center of New Hampshire. Please take a moment to review the following information about your upcoming visit with us.

Prior To Your Appointment

In order to save you time at your appointment, we encourage you to download and complete our new patient packet.

If you are transferring care from an outside practice, we ask that you request your records and bring them with you to your visit. Medical records can also be faxed to (603) 665-9360.

The Day of Your Visit

Please be sure to bring the following to your appointment:

• Completed New Patient Packet (including a contact lens waiver if you are having a contact lens exam)
• Insurance card
• If you are covered by an HMO, the appropriate referral authorization forms and/or numbers
• Driver’s license or photo ID
• Copayment
• Current prescription eyeglasses and/or contact lense
